Abstract听听

Most public health professionals are familiar with the foundational ideas of Geoffrey Rose, the influential British epidemiologist (1970s-1990s). Rose鈥檚 concepts of 鈥溾�� versus 鈥溾�� are widely cited in most current policy and practice documents about chronic disease control internationally. Less widely understood are the specific pros and cons of each approach; how they are generally complementary rather than mutually exclusive options; why upstream prevention, despite its strengths, is underused 鈥� and what can be done about it. Prof. Frank will compare two diseases that clearly demonstrate these principles of prevention, illustrating the strengths and weaknesses of upstream and downstream approaches:听

• The historical epidemic of pellagra in the southern USA, 1900-1940;听

• The current obesity pandemic, well established in high-income countries and now spreading quickly to middle- and lower-income settings.听

Speaker Bio听

Professor John Frank is a physician-epidemiologist with 46 years of primary care/ public health practice, research, teaching and consulting, in four countries, with a specific research interest in health inequalities by socio-economic status. During his career, he led the development of three applied public health research institutes: the Institute for Work & Health (Toronto); the Canadian Institutes of Health Research - Institute of Population and Public Health; and the Scottish Collaboration for Public Health Research and Policy. He was a professor at the Dalla Lana School of Public Health (University of Toronto) and Usher Institute (University of Edinburgh). Professor Frank trained in Medicine and Community Medicine at the University of Toronto, in Family Medicine at McMaster University, and in Epidemiology at the London School of Hygiene and Tropical Medicine.听

Les id茅es de l鈥檌nfluent 茅pid茅miologiste britannique Geoffrey Rose, qui a jet茅 certains des fondements de la discipline dans les ann茅es 1970 脿 1990, sont tr猫s connues dans le milieu de la sant茅 publique. Ses concepts et sont tr猫s cit茅s dans la documentation actuelle sur les politiques et les pratiques de lutte contre les maladies chroniques 脿 l鈥櫭ヽhelle internationale. Les avantages et inconv茅nients de chaque approche et le fait qu鈥檈lles sont g茅n茅ralement compl茅mentaires, plut么t que mutuellement exclusives, sont toutefois mal compris. On conna卯t 茅galement peu les raisons pour lesquelles la pr茅vention en amont est sous-utilis茅e, en d茅pit des atouts de cette strat茅gie 鈥� et ce qu鈥檕n peut faire pour y rem茅dier. Le Pr Frank comparera deux maladies qui illustrent clairement ces principes de pr茅vention ainsi que les forces et faiblesses des approches en amont et en aval :

• l鈥櫭﹑id茅mie de pellagre v茅cue dans le sud des 脡tats-Unis de 1900 脿 1940;
• l鈥檃ctuelle pand茅mie d鈥檕b茅sit茅, qui est bien 茅tablie dans les pays 脿 revenu 茅lev茅 et se propage rapidement dans les pays 脿 revenu faible et interm茅diaire.

Biographie

M茅decin 茅pid茅miologiste, le professeur John Frank compte 46 ann茅es de pratique, de recherche, d鈥檈nseignement et de consultation en soins de premi猫re ligne et en sant茅 publique dans quatre pays. Il s鈥檌nt茅resse particuli猫rement aux in茅galit茅s en mati猫re de sant茅 en fonction du statut socio-茅conomique. Au cours de sa carri猫re, il a dirig茅 la cr茅ation de trois instituts de recherche appliqu茅e en sant茅 publique : l鈥橧nstitut de recherche sur le travail et la sant茅 (Toronto), l鈥橧nstitut de la sant茅 publique et des populations des Instituts de recherche en sant茅 du Canada et la Scottish Collaboration for Public Health Research and Policy. Il a 茅t茅 professeur 脿 la Dalla Lana School of Public Health (Universit茅 de Toronto) et 脿 l鈥橴sher Institute (University of Edinburgh). Le Pr Frank a suivi sa formation en m茅decine et en m茅decine communautaire 脿 l鈥橴niversit茅 de Toronto, en m茅decine de famille 脿 l鈥橴niversit茅 McMaster et en 茅pid茅miologie 脿 la London School of Hygiene and Tropical Medicine.
